We are seeking a highly skilled and compassionate Care Coordinator to join our team at The Center for Youth and Family Solutions. As a Care Coordinator, you will play a critical role in organizing, facilitating, and managing the coordination of care for referred youth involved with complex behavioral health concerns.
Key Responsibilities- Organize a spectrum of effective, community-based services and supports for children and youth with or at risk for mental health or other challenges and their families.
- Cultivate and support provider and stakeholder relationships on identified cases with timely communication, inquiry, follow-up, and integration of information into the care plan.
- Complete needed psychosocial and mental health assessments to determine treatment recommendations.
- Organize and facilitate Child and Family Team (CFT) meetings among providers involved with clients for a culture of respect, responsibility, and hope.
- Assist the teams in developing and coordinating treatment and supportive services and in assessing treatment progress.
- Conduct community engagement and provide services to the child and family in home or other community-based locations.
- Maintain records and reporting in accordance with HFS requirements and/or Medicaid billing requirements.
- Bachelor's degree in Social Work, Human Service, or related field.
- Mental Health or Human Service Experience (preferred).
- Completion of DCFS criminal background check and fingerprinting.
- Must be at least 21 years of age.
- Obtain an insured vehicle.
- At least three years of driving experience with a valid driver's license.
- Interpersonal Skills.

- Bilingual (Not Required).
