Key Responsibilities- Conduct thorough health assessments, including medical history, diagnosis, and treatment planning
- Develop and implement personalized care plans to address patients' unique needs and goals
- Collaborate with healthcare providers, case managers, and other stakeholders to ensure seamless care coordination
- Provide education and support to patients and their families to promote healthy behaviors and self-management
- Document all assessments and care plans in accordance with Matrix Medical Network's guidelines and protocols
- Master's degree in Nursing or related field
- Current RN and NP licensure in the state of practice, with prescription authority or ability to obtain
- Board certification in a Matrix-approved specialty
- Minimum of 1 year experience as a Nurse Practitioner
- Strong computer skills and familiarity with electronic health records
- Excellent communication and interpersonal skills
