Job Title: Physical Therapist PRN

We are seeking a skilled Physical Therapist to join our team at Continuous Home Care. As a Physical Therapist, you will play a vital role in providing high-quality care to our geriatric patients in the comfort of their own homes.

Responsibilities:
  • Assess patient's physical therapy needs and collaborate with physicians to evaluate level of function.
  • Develop and implement plans of care tailored to assessment findings.
  • Provide physical therapy to patients in their homes or community settings.
  • Teach and instruct patients on active and passive exercises.
  • Treat patients to alleviate pain and/or develop or restore function to maintain maximum performance.
  • Evaluate patients for adaptive equipment needs and obtain physician orders for same.
  • Arrange for necessary adaptive equipment.
  • Educate patients and caregivers on the use of assistive devices.
  • Document all patient care on appropriate forms/patient's medical records.
  • Submit completed documentation within established time parameters.
  • Act as a resource to management and field staff.
  • Participate in multidisciplinary case conferences and staff meetings.
  • Demonstrate a high degree of commitment to customer service and quality care.
  • Supervise Physical Therapy Assistants and Home Health Aides as needed.
  • Participate in utilization review and quality improvement activities.
  • Review and recommend physical therapy policies and procedures.
Requirements:
  • Graduation from a physical therapy program accredited by the Pennsylvania Commission on Accreditation in Physical Therapy Education or a national organization recognized by the Board.
  • Active license through the State of Pennsylvania.
  • A minimum of one year of institutional physical therapy experience.
  • Previous home healthcare experience is welcome but not required.
