About the Role

Amedisys, a leading provider of home health and hospice services, is seeking a skilled Hospice Liaison to join our team. As a key member of our sales team, you will be responsible for developing and maintaining relationships with healthcare professionals, patients, and families to promote our home care services.

Key Responsibilities
  • Business Development: Educate healthcare professionals, patients, and families about our home care services and develop relationships with key referral sources, such as hospitals, nursing homes, and physician offices.
  • Patient Admissions: Coordinate patient admissions with our team, including obtaining all required documentation to complete the referral to admission process.
  • On-Site Visits: Conduct on-site visits to assess patient needs and identify barriers to care.
  • Record Keeping: Maintain accurate records of patient interactions and referrals.
  • Goal Achievement: Achieve admissions, product, and payor-mix goals, as well as meet short and long-term target account goals.
Requirements
  • Experience: At least one year of experience in a healthcare-related business development or outreach role.
  • Education: A bachelor's degree is preferred, but not required.
  • Transportation: Reliable transportation, a current driver's license, and liability insurance are required.
