Job SummaryThe Case Manager will be responsible for conducting social service assessments and connecting HIV+ individuals with needed services offered by Federal, State, and Local community programs. This is an exciting opportunity to make a meaningful difference in the lives of those affected by HIV/AIDS.
Responsibilities- Conduct social service assessments to identify the needs of HIV+ individuals
- Connect individuals with needed services, including medical care, housing, and employment assistance
- Develop and implement individualized service plans to meet the unique needs of each client
- Provide ongoing support and advocacy to ensure clients receive the services they need
- Collaborate with community partners to ensure seamless service delivery
- Bachelor's degree in Social Work or a related field
- Basic knowledge of medical case management principles
- Experience working in a nonprofit organization or a strong desire to work in a service-driven environment
- 100% commitment to Matthew 25's mission, vision, and values
- Flexibility to travel to various locations in Kentucky
- Ability to pass a drug screen and background test
