Job Summary:

We are seeking a skilled Speech Language Pathologist to join our team at HarmonyCares, providing high-quality speech therapy services in the comfort of patients' homes. As a Speech Language Pathologist, you will be responsible for delivering comprehensive assessment, evaluation, and treatment plans tailored to each patient's unique needs.

Your Role:
You will work collaboratively with our interdisciplinary team to develop and implement individualized patient care plans, incorporating your expertise in speech therapy to address specific communication needs. Key responsibilities include:
  • Providing speech therapy services, including assessment, evaluation, procedures, teaching, and training activities as outlined in the patient care plan
  • Documenting daily changes and updates to patient care plans, including discharge planning
  • Attending staff meetings, team conferences, and educational in-services as required by agency standards
  • Performing all OASIS time point assessments per Medicare criteria and submitting recertification paperwork according to policy and procedure

Requirements:
To succeed in this role, you will need:
  • A minimum of one year's experience as a home care Speech Language Pathologist, with proficiency in performing comprehensive assessments and OASIS-C
  • The ability to assess and document patient needs, formulating individualized patient care plans that meet those needs
  • Proficiency in Microsoft Office

Estimated Salary Range:
$90,000 - $110,000 annually
