Job Summary:

Responsibilities:

PRIMARY RESPONSIBILITIES

Ensure safety and appropriateness of medication therapy through order verification, dispensing, home medication reconciliation, chart review, patient counseling, lab value monitoring, and dose/therapy adjustment as applicable.

Document medication-related plans and interventions in the electronic medical record.

Provide clinical consultation, delivering best-practice, evidence-based, and complete information to practitioners.

Make recommendations and encourage safe and cost-effective pharmacotherapy for patients through multi-disciplinary collaboration.

Oversee the medication dispensing process to ensure accurate and timely delivery of optimal pharmacotherapy to patients.

This includes supervision of pharmacy technicians and/or interns who support the role of the pharmacist in the drug delivery system.

Precept pharmacy students and residents, as applicable.

Work in a constant state of alertness and safe manner.

Perform other duties as assigned.


Requirements:

EDUCATION
Pharmacy degree (Pharm.D; Bachelor of Pharmacy) from an accredited school of pharmacy

EXPERIENCE
No experience required

PHYSICAL REQUIREMENTS
Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
Frequent sitting, standing, walking, reaching, and repetitive foot/leg and hand/arm movements.

Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.

Frequent use of hearing and speech to share information through oral communication. Ability to hear alarms, malfunctioning machinery, etc.
Frequent keyboard use/data entry.
Occasional bending, stooping, kneeling, squatting, twisting, and gripping.
Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
Rare climbing.
