Job Summary:

We are seeking a highly skilled Senior Patient Care Nurse to join our team at Infojini. In this role, you will be responsible for delivering exceptional patient-centered nursing and rehabilitation services in a subacute long-term care/rehab facility.

Main Responsibilities:

  • Collaborate with the Unit Manager to maintain outstanding clinical standards and patient experience.
  • Provide high-quality service to all patients, families, and visitors, ensuring their satisfaction.
  • Ensure timely administration of medications, treatments, and nursing care as ordered.
  • Complete accurate documentation in accordance with state/federal guidelines.

Essential Job Functions:

  1. Supervise and assign nursing personnel on the unit, promoting effective teamwork.
  2. Establish comprehensive nursing care plans, utilizing nursing diagnoses in cooperation with the supervisor and nursing staff.
  3. Implement individualized plans of care for residents, prioritizing their needs.
  4. Recognize psychosocial needs and intervene to provide emotional support, enhancing patient well-being.
  5. Maintain accurate, informative records on all patients in the unit.
  6. Identify and report any real or potential problems regarding patient care.
  7. Manage inventory of medications, equipment, and supplies on the unit.
  8. Record, report, and interpret patient responses to treatment and care to relevant team members.
  9. Respond to changes in patient conditions/needs and notify the Charge nurse.
  10. Maintain equipment according to facility policy and use it consistently with specifications.
  11. Represent Infojini and Nursing staff on committees, as requested by supervisors.
  12. Foster good relationships and communication with Team Members, patients, family/significant others, and visitors.
  13. Assist with new Team Member orientation and provide key experiences for independent performance.

Required Skills & Experience:

  • Demonstrate a collaborative approach to problem-solving through frequent communication and teamwork.
  • Minimum 1-year prior experience working as an RN in a subacute long-term care/rehab facility.
  • Experience working with various patient conditions and ages is essential.

Education:

  • ASN (Associate Degree in Nursing) from an accredited school of nursing is required.
  • BSN (Bachelor of Science in Nursing) is preferred but not mandatory.

Certifications & Licensure:

  • BLS - AHA certification is required.
  • NJ State Professional Registered Nurse license or multi-state compact license is also required.
