Job Summary

CenterWell Home Health is seeking a skilled Registered Nurse to join our team on a per diem basis. As a Home Health Nurse, you will provide high-quality patient care and support to individuals in the comfort of their own homes.

Key Responsibilities
  • Provide skilled nursing visits to patients in their homes, including admission, case management, and follow-up care.
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
  • Confer with physicians to develop and revise care plans as needed.
  • Coordinate care with other healthcare professionals, including physical therapists, occupational therapists, and home health aides.
  • Document patient care and progress in accordance with regulatory requirements.
Requirements
  • Diploma, Associate, or Bachelor's degree in Nursing.
  • Minimum one year of nursing experience, preferably in home health or a related field.
  • Current and unrestricted Registered Nurse licensure.
  • Current CPR certification.
  • Strong organizational and communication skills.
