About the Job:

We are seeking a skilled Physical Therapist Assistant to join our team at All Medical Personnel.

Job Summary:

The successful candidate will work collaboratively with healthcare professionals to provide high-quality physical therapy services to patients in their homes. As a Physical Therapist Assistant, you will be responsible for assessing patients, developing personalized treatment plans, and implementing interventions to promote optimal health and well-being.

Salary and Benefits:

We offer an attractive salary of $65,000 - $85,000 per year, depending on experience, as well as a comprehensive benefits package including medical, dental, and vision insurance, 401(k) retirement plan, and paid time off.

Responsibilities:

  • Collaborate with interdisciplinary teams to develop holistic plans of care focused on achieving patient goals.
  • Instruct, coach, guide, and intervene to support patients and families in achieving best possible outcomes.
  • Promote optimal health, well-being, and quality of life of home health patients and their families by providing skilled, intermittent physical therapy.
  • Practice independently in patient homes using excellent evidence-based practice for assessment, clinical decision making, clinical practice, and coordination of care across the continuum.
  • Act as case manager and team lead for patients requiring home health therapy services when nursing is not involved or physical therapy is primary patient need.
  • Assess patients and collaborate with interdisciplinary teams and primary care providers to develop patient-centered plans of care.
  • Provide physical therapy treatment to patients/families/caregivers as ordered.
  • Complete accurate clinical documentation to effectively support service provided within specified timeframes.
  • Provide physical therapy to pediatric, adolescent, adult, and geriatric patients based on principles of growth and development and life stages.
  • Maintain communication regarding patients with case managers and other disciplines via care conferences or other appropriate methods.
  • Report acute medical problems and condition changes to primary care providers.
  • Provide supervision to Physical Therapy Assistants, Home Health Aides, and students in clinical settings.
  • Complete regulatory required assessments within specified timeframes.
  • Complete medication reconciliation at specified timeframes.

Requirements:

  • 1-2 years outpatient rehabilitation setting experience.
  • At least two (2) years of physical therapy experience highly preferred.
  • One (1) year experience in home health or other health setting with geriatric emphasis.
  • Current license in Physical Therapy or ability to obtain prior to hire.
  • American Heart Association Healthcare Provider BLS preferred upon hire, and required within 90 days of hire.
  • Current Driver?s license.
  • Excellent customer service skills.
  • Ability to work without direct supervision.
  • Care appropriate to the age of patients served on the job.
  • Knowledge of physical therapy assessment and treatment methods.
  • Knowledge of physical therapy modalities and equipment.
  • Ability to maintain effective relationships with other healthcare professionals.
  • Must be able to develop skills in use of technology used in home care setting.
  • Demonstrate the highest level of professionalism when interacting with patients, families, physicians, and hospital staff members.
