Key Responsibilities:- Provide direct patient care under the supervision of a registered nurse, following the plan of care and implementing treatments as defined in the State's Nurse Practice Act.
- Assess and provide patient and family/caregiver education and information pertinent to diagnosis and plan of care.
- Participate in coordination of home health services, reporting identified needs for other disciplines to the registered nurse and/or Clinical Supervisor/Nursing Supervisor.
- Use equipment and supplies effectively and efficiently.
- Participate in personal and professional growth and development.
- Perform on-call rotation and other duties as assigned by the registered nurse.
- Graduate of an accredited practical nurse or vocational nursing program.
- Minimum of one year nursing experience required.
- Experience in the home health industry is preferred.
- Currently licensed as a licensed practical nurse or licensed vocational nurse in the state.
- Complies with accepted professional standards and practice.
- Demonstrates good verbal and written communication, and organization skills.
- Must be a licensed driver with an automobile that is insured in accordance with state and organization requirements and is in good working order.
- Possesses and maintains current CPR certification.
- Duties require extensive standing and walking.
- Duties require reaching, twisting, stooping, bending, kneeling, and crouching.
- Requires lifting 50 lbs or more, positioning, pushing, and/or transferring patients.
- Sitting and driving up to 3 hours per day.
- Ability to drive safely.
