Job Title: Medication Care Manager

Responsibilities:
  • Administer medications and treatments in a safe and organized manner.
  • Review and document medication administration records.
  • Communicate with residents, families, and healthcare professionals to ensure accurate and timely medication administration.
  • Participate in the development and implementation of individualized care plans.
  • Collaborate with the care team to ensure seamless delivery of care services.
Requirements:
  • High school diploma or equivalent required.
  • Certifications in medication administration and management preferred.
  • Previous experience working with seniors and in a healthcare setting preferred.
  • Ability to work in a fast-paced environment and prioritize tasks effectively.
  • Excellent communication and interpersonal skills.
