About the Job

We are seeking a skilled Registered Nurse to join our Home Health team at Baylor. As a Home Health Registered Nurse, you will play a vital role in providing high-quality patient care and support to individuals in their homes.

Job Summary

The successful candidate will be responsible for providing admission, case management, and follow-up skilled nursing visits for home health patients. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.

Key Responsibilities
  • Provide admission, case management, and follow-up skilled nursing visits for home health patients.
  • Develop and implement individualized care plans that meet the unique needs of each patient.
  • Confer with physicians to develop and update patient care plans.
  • Provide hands-on care, management, and evaluation of patient care plans.
  • Coordinate care with other healthcare professionals, including physical therapists, occupational therapists, and home health aides.
  • Report patient care and progress to physicians and other healthcare professionals.
  • Implement patient care plans in conjunction with patients and families to achieve optimal outcomes.
Requirements
  • Diploma, Associate, or Bachelor's degree in Nursing.
  • Minimum of one year of nursing experience, preferably in a home health setting.
  • Strong med-surg, ICU, ER, and acute care experience.
  • Current and unrestricted Registered Nurse licensure.
  • Current CPR certification.
  • Strong organizational and communication skills.
