About the RoleWe are seeking an experienced or newly graduated Physical Therapist (PT) to join our growing team for a contract assignment. The successful candidate will be responsible for assessing referred patients who require rehabilitation services, including providing direct patient care and establishing treatment plans tailored to each individual's needs.
The ideal candidate will have graduated from an accredited school and hold a current state license as a Registered Physical Therapist/PT. A pro-active approach to obtaining licensure is also highly valued.
Key Responsibilities- Develop a plan of care for each physical therapy patient
- Provide skilled physical therapy services/interventions in accordance with physician orders
- Assess patient needs, plan for, evaluate and modify care to meet goals of physical therapy interventions
In addition to your clinical expertise, you will also be required to document and record patient progress, educate patients and families on appropriate physical therapy methods, and collaborate with healthcare professionals to ensure seamless care coordination.
