Responsibilities- Provide comprehensive physical therapy services to patients in their homes, including assessment, evaluation, and treatment planning.
- Communicate effectively with patients, families, and other healthcare professionals to ensure seamless care coordination.
- Supervise and instruct non-professional home health aide staff in the provision of personal care to patients.
- Update care plans regularly to reflect changes in patient needs and progress.
- Submit completed visit notes and participate in ongoing quality improvement initiatives.
- Current, unencumbered State Physical Therapy License in the state of practice.
- One year of experience as a home care professional Physical Therapist, with preference given to those with OASIS-C experience.
- Ability to make sound professional clinical judgment and assess patient needs.
- Proficiency in clinical skills, Microsoft Office, and ability to drive in a car for 2-4 hours per day.
- Bonus potential of $2500+ per quarter.
