Position Summary

Under the guidance of Nursing Administration, the Patient Care Coordinator plays a vital role in coordinating comprehensive healthcare for individuals and families. This involves assessing needs, developing care plans, facilitating access to resources, and advocating for optimal patient outcomes.

Patient Care Coordinators collaborate with various healthcare professionals and community partners to ensure seamless transitions of care and promote quality, cost-effective services. They also contribute to maintaining nursing skills and competencies by participating in clinical activities within other departments as needed.

Knowledge/Skills/Abilities
  • Demonstrates strong organizational and communication skills.
  • Provides safe and effective patient care aligned with established nursing standards.
  • Exhibits sound judgment and problem-solving abilities.
  • Communicates professionally and respectfully with all stakeholders.
Supervision Received

Reports to the Director of Nursing or their designee.

Licensure/Certification/Registration
  • Current Registered Nurse License (any state), Alaska Registered Nurse License required within six months of hire.
  • Current BLS Certification
  • Current ACLS Certification (for LPN, ACLS preferred)
  • PALS, NRP, TNCC, ENPC Preferred
  • Case Management Certification Preferred
Education

Graduate of an Accredited School of Nursing

Experience

Demonstrates a clear understanding of nursing theory and practice. Three to five years of experience in Emergency care is preferred.

Orientation Period

Based on experience and ongoing performance evaluation by Nursing Administration. Orientation includes formal department training and a minimum of 2-4 days during the first two weeks of employment.
