We are seeking a part-time Palliative Care Advanced Practice Provider to join our team at Bay Harbor Post Acute and Healthcare Center in Salisbury, MD.

Job Summary

The Palliative Care Advanced Practice Provider will provide primary and urgent care services to nursing center residents in collaboration with the primary care team. Emphasis will be on the care of the individual patient and family with a focus on pain and symptom management, psychosocial support, and advance care planning.

Responsibilities
  • Provide palliative care services to nursing center residents with serious illness
  • Collaborate with the primary care team to develop and implement individualized care plans
  • Provide guidance and education to patients, families, and healthcare teams on palliative care principles and practices
  • Participate in interprofessional team meetings to discuss patient care and develop strategies for improving patient outcomes
Requirements
  • Graduate from an accredited School of Nursing program and licensed as a registered nurse
  • Certified as a Nurse Practitioner by being a nursing graduate from an accredited Nurse Practitioner Program with a Master's Degree in Nursing and a concentration and specialty track of Older Adult and/or Family Nurse Practitioner
  • PA state licensure required for Physician Assistants
  • A minimum of two years experience working with the adult or geriatric population preferred
