Job Title: Case Management Specialist

At Inova, we are seeking a highly skilled Case Management Specialist to join our team. As a key member of our Case Management department, you will play a critical role in ensuring that our patients receive the highest level of care and support.

Key Responsibilities:
  • Coordinate and perform administrative activities to support the Case Management department.
  • Participate in the annual and ongoing budget processes.
  • Arrange services for assigned patient populations, including Home Health, Durable Medical Equipment, Ambulance, Housing, Skilled Nursing Facility, and others.
  • Communicate and document accurate clinical/financial information in both verbal and written forms.
  • Deliver and document delivery of regulatory forms, such as the mandated Observation letter and Medicare IMM notifications.
  • Receive and document benefit and contract level requirements.
  • Conduct timely arrangements of services to meet customer requirements.
  • Address and solve provider and payor issues quickly.
  • Distribute and document delivery of mandated Observation and Medicare IMM/HINN letters as directed.
  • Communicate financial and clinical information to clinicians, patients, families, care team members, and external customers.
  • Document clinical and financial patient information to assist with discharge, planning, and access to continuing care services.
  • Communicate with payor and document detailed instructions on billing, authorization, and utilization of services.
  • Complete state and federal forms related to patient level of care, according to regulatory standards.
  • Manage and maintain state required UAI filings as well as all accounts receivable/payable functions for the department.
  • Provide support to the strategic planning and business development initiatives of the department.
  • Review and approve time/attendance for department staff on a biweekly basis.
  • Perform periodic reviews to track employee time and attendance for department leadership.
  • Submit any identified time/pay adjustments to Payroll in a timely manner.
  • Maintain records of all system Case Management files and act as liaison to Foundation, auxiliary, and external vendors.
  • Assist in monitoring the operational budget, in collaboration with department Director, related to funds disbursement and reconciliation.
  • Track and monitor accounting for budgets, capital requests, travel funds, and business expenses.
Requirements:
  • 2 years of relevant experience.
  • High School or GED.
