Job Summary

We are seeking a compassionate and skilled Pediatric Home Health Nurse to join our team. As a Pediatric Home Health Nurse, you will be responsible for providing one-on-one care to our patients in their homes, working collaboratively with our interdisciplinary team to develop and implement individualized care plans.

Responsibilities
  • Provide skilled nursing care to pediatric patients in their homes, including wound care, medication management, and vital sign monitoring.
  • Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing strategies for addressing these needs.
  • Collaborate with our interdisciplinary team, including physicians, therapists, and social workers, to develop and implement individualized care plans that meet the unique needs of each patient.
  • Communicate effectively with patients, families, and caregivers to ensure seamless care coordination and address any concerns or questions they may have.
  • Participate in ongoing education and training to stay current on best practices in pediatric home health care.
Requirements
  • RN/LPN with a current valid license to practice as issued by the State Board of Nursing.
  • Current CPR certification.
