LHC Group is seeking a highly skilled and compassionate Home Health Care Professional to join our team as a Patient Care Manager Assistant in Marshall, Texas.

About the Role

This is an exciting opportunity to work in a dynamic environment where you will be responsible for assisting the Patient Care Manager in coordinating a team of staff to ensure high-quality care to home health patients. You will play a vital role in ensuring that the team provides quality care in an efficient manner, adhering to physician's orders and State/Medicare guidelines.

Responsibilities
  • Assist the Patient Care Manager in coordinating patient care and ensuring continuity of service.
  • Prepare patient lists and necessary documents for weekly case conferences.
  • Provide direct patient care, as needed, in accordance with your scope of practice, plan of care, and physician orders.
  • Assist in clinician documentation review/audits to ensure complete, accurate, and timely submission of all documentation by field staff.
Requirements
  • CPR certification
  • Current LPN licensure in the state of practice
  • Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
