Job Summary

We are seeking a highly skilled Occupational Therapist to join our team at OLV Human Services. As an Occupational Therapist, you will provide occupational therapy services to individuals in accordance with their Individual Education Plan or Individual Family Service Plan.

Key Responsibilities
  • Review all available documentation related to the individual prior to beginning treatment
  • Develop an appropriate treatment plan for each individual
  • Perform treatments according to IFSP/IEP and accepted professional standards and practices
  • Conduct ongoing assessment of individual during each visit and modify goals/outcomes/treatment plan accordingly
  • Perform appropriate parent/caregiver education and develop home programs as necessary
  • Work as part of an inter-disciplinary team, utilizing communication with all team members to enhance the treatment program of the individual
  • Maintain an accurate schedule
  • Submit all required documentation of visits according to established procedures
  • Perform evaluations as assigned and when necessary
  • Communicate with families on a regular basis
  • Keep abreast of legal responsibilities, professional ethics, standards of practice and agency rules, policies and regulations
  • Provide for own professional growth by attending continuing education and in-services; tracking this to assure compliance with state licensing requirements
  • Discontinue therapy when child no longer meets eligibility requirements
  • Provide own transportation to service locations
  • Maintain client confidentiality with HIPPA/FERPA and agency guidelines
  • Maintain professional appearance, follows agency dress code
  • Attend annual fee for service meeting and any other meetings required by your supervisor
  • Attend CPSE and IFSP meetings for individuals on caseload as required
  • Adhere to and signs Annual Employee Agreement
  • Document time off and time worked accurately according to agency policies
  • Perform other duties and assumes other responsibilities as assigned by their direct supervisor
Requirements
  • Graduate of an accredited Occupational Therapy program with a Bachelor's Degree
  • Minimum 1-2 years of pediatric experience required
  • New York State Occupational Therapy License and Registration
  • Valid NYS Driver's License
  • Minimum of 36 hours of Continuing education per licensing period (3 years) as per NYS Office of Profession guidelines
  • All required agency trainings
  • All trainings required by programs you service individuals in, including but not limited to, OCFS and Early Intervention
Physical Requirements

The frequent physical demands of the position are sitting, standing, walking, climbing, talking, listening, repetitive motions, lifting, carrying, pushing, pulling, balancing, stooping, crouching, reaching, handling of children and grasping.

All services rendered will be paid at the Fee For Service (FFS) rate, per the service provided.
