Job Title: Advanced Practice Provider - Shadow Creek Clinic

About the Role

We're seeking a highly skilled Advanced Practice Provider to join our team at Shadow Creek Clinic. As an Advanced Practice Provider, you will be responsible for providing high-quality, patient-centered care to our patients.

Responsibilities:

  • Provide comprehensive or episodic health history and physical assessment of patients/families with acute, chronic, and/or potential health problems
  • Develop and implement individualized treatment plans to meet the unique needs of each patient
  • Collaborate with other healthcare professionals to ensure seamless care coordination
  • Stay up-to-date with the latest medical research and best practices to provide evidence-based care
  • Communicate effectively with patients, families, and healthcare teams to ensure clear understanding and expectations

Requirements:

  • Graduate and certified from an accredited advanced practice program
  • Two or more years of outpatient clinical experience as an Advanced Practice Provider, with a demonstrated ability to work autonomously
  • Current, unrestricted license in practicing state or be eligible for licensure
  • Active Drug Enforcement Agency (DEA) registration or be eligible for registration
  • Must possess active Medicare/Medicaid Enrollment or be eligible to enroll
  • Certified in Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS)
  • Strong oral and written communication skills
  • Comfortable with Electronic Medical Record (EMR) system; Athena preferred
  • Familiarity with Microsoft Office
