Job Summary: We are seeking a skilled PRN Physical Therapy Assistant to join our team at Edenbrook of Green Bay. As a key member of our rehab department, you will play a vital role in providing high-quality therapy services to our patients.
Key Responsibilities:
- Maintain the highest standards for rehabilitation services, aligning with our company's mission and goals
- Demonstrate exceptional patient care, ensuring a comfortable and respectful experience for all individuals
- Deliver quality therapy interventions, including treatment planning, program implementation, and discharge planning, under the supervision of a licensed physical therapist
- Prioritize your work schedule to meet the needs of our facility and collaborate effectively with our therapy management staff
- Foster a positive and productive working relationship with all facility staff, promoting a collaborative environment
- Adhere to established policies and procedures, maintaining a smooth day-to-day operation of our rehab department
- Screen patients to determine the need for evaluation, under the supervision of a physical therapist
- Treat patients to relieve pain and restore function, maximizing performance outcomes under the guidance of a physical therapist
- Monitor patient response to intervention, communicate changes, and update treatment modalities as needed, under the direction of a physical therapist
- Meet documentation requirements in a timely and accurate manner
- Collaborate with other team members regarding patient care and communicate effectively to ensure seamless service delivery
- Assist with data collection on patient progress, under the supervision of a physical therapist
- Coordinate patient discharge when maximum benefits have been achieved, in conjunction with a physical therapist
- Provide accurate and complete billing summaries as required
- Participate in in-service education sessions, staying up-to-date on best practices and industry developments
- Maintain a clean and organized therapy department, reflecting positively on our facility
- Perform administrative duties to support the provision of therapy services
- Identify new rehab admissions and notify the physical therapist as necessary
- Communicate upcoming patient discharges with facility staff, as required
- Attend care plan meetings, as requested, and participate in ongoing professional development
- Contribute to in-service education sessions for staff, sharing expertise and knowledge
- Supervise physical therapist assistant students, as appropriate
