Senior Care Physician Lead

We are looking for a compassionate and skilled Primary Care Physician to lead our team at CenterWell Senior Primary Care. As a valued member of our organization, you will have the opportunity to make a positive impact on the lives of seniors in our community.
Responsibilities:- Provide primary care services to seniors, including routine check-ups, chronic condition management, and preventive care
- Develop and maintain relationships with patients and their families to ensure high-quality care
- Collaborate with healthcare professionals to coordinate patient care and services
- Participate in quality improvement initiatives to enhance patient satisfaction and health outcomes
To be considered for this role, you will need:
- A Doctor of Medicine (M.D.) or Doctor of Osteopathic Medicine (D.O.) degree from an accredited university
- Current and unrestricted medical license in the state of practice
- Board certification in Family Medicine or Internal Medicine (or eligibility)
- At least 2 years of relevant experience in primary care
