About Our Job

We are seeking a highly motivated and compassionate individual to join our team as a Housing Case Management Stabilization Outreach Case Coordinator. As a key member of our department, you will play a critical role in supporting Denver residents as healthy, housed, and connected.

What You'll Do

As a Housing Case Management Stabilization Outreach Case Coordinator, you will provide initial, introductory, professional engagement of people experiencing unsheltered and sheltered homelessness to facilitate connection to continuum housing and homelessness services. You will develop professional and supportive relationships with people experiencing homelessness, identify and document client needs and services provided in Homeless Management Information System (HMIS), and provide service information and direct transportation as needed to contacted individuals.

Required Minimum Qualifications
  • Education requirement: Graduation from high school or the possession of a GED, HiSET, or TASC Certificate.
  • Experience Requirement: Four (4) years of experience performing intake, including assessing and identifying client strengths and needs and providing referrals for assistance.
  • Education/Experience Equivalency: Additional appropriate education may be substituted for the minimum experience requirement.
  • License/Certifications: Requires a valid Driver's License at the time of application.
