  • Minimum 1 year of recent Primary Care experience as a Nurse Practitioner or Physician Assistant.
  • Working knowledge of quality measurement in the management of chronic disease conditions.
  • Effective verbal, written, and electronic communication skills.
  • Outstanding organizational skills and ability to multi-task.
