We are seeking a compassionate and experienced Registered Nurse to supervise medical services within the Senior Resources Adult Day Care Center.

As a Registered Nurse for Seniors, you will be responsible for ensuring the physical and emotional well-being of our elderly patients.

This position provides a wide range of comprehensive medical services mandated by the Iowa Department of Inspections and Appeals, the Department of Human Services, as well as local health oversight bodies, funders, family members, and other interested stakeholders of the Senior Resources program.

Responsibilities:
  • Coordinating and/or delegating treatment
  • Health screenings and monitoring of participant status
  • Providing general health and nursing guidance of services for staff
  • Administering medications and treatments as prescribed by physicians
  • Monitoring and recording vital signs and medical information
  • Collaborating with an interdisciplinary team to develop and implement comprehensive care plans
  • Providing emotional support and comfort to patients and their families
  • Educating patients and families on health management and disease prevention
  • Maintaining accurate and up-to-date medical records of all services provided
Essential Functions:
  • Maintaining knowledge of current rules and regulations covering Adult Day Centers in Iowa as provided by Iowa Administrative Code, the Department of Inspections and Appeals, The Department of Human Services, local community standards, and accepted best practices for Nursing.
  • Competently assessing potential and current participants in the Adult Day Center using the Mini Mental Status Exam (MMSE), the Global Deterioration Scale (GDS), the Functional Assessment, The Departments Nursing Review (Initial, Annual, and 90 Day) and direct observation via Nursing notes and documentation.
  • Distributing information about patient health status and concerns with program staff, as well as other providers outside of the agency involved in the participant care.
  • Assisting in the development of care plans designed to reflect the choices of the participant or their POA/guardian, as well as provide optimal medical treatment for participants in the Adult Day Center.
  • Providing the initial intake screening and assessment process of ADC participants.
  • Providing 90 day Nurses review status and services for ADC participants.
  • Interfacing with outside healthcare professionals (e.g. Physicians, Health Care Professionals, Case Managers) to coordinate services, to ensure that medications are correct, to receive general health status reports, to receive clarity on Physician's orders, to ensure that services are needed and not duplicated, and to generally ensure that the best care is provided to the participant.
  • Maintaining accurate and effective documentation of services provided to each Adult Day Center participant.
  • Assisting in monthly audit of client files to ensure all health care information is up to date and relevant.
  • Effectively adhering to HIPAA standards and procedures.
  • Providing required in services to ADC staff members, as well as in service topics that may enhance the medical services of the Adult Day Center.
  • Requirements:
    • Active Iowa Registered Nurse license
    • Experience in geriatric nursing is preferred
    • Strong communication and interpersonal skills
    • Ability to work independently and as part of a team
    • Knowledge of medical terminology and procedures
    • Familiarity with electronic medical records
