Home Health Registered Nurse

We are seeking a compassionate and skilled Registered Nurse to join our team in Irvine. As a Home Health and Hospice Registered Nurse, you will be responsible for delivering comprehensive nursing care to patients in their homes.
Responsibilities:- Conduct initial assessments of patient and family to determine home care needs.
- Provide a complete physical assessment and history of current and previous illness(es).
- Regularly re-evaluate patient nursing needs.
- Initiate the plan of care and make necessary revisions as patient status and needs change.
- Use health assessment data to determine nursing diagnosis.
- Develop a care plan which establishes goals, based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions.
- Initiate appropriate preventive and rehabilitative nursing procedures.
- Administer medications and treatments as prescribed by the physician.
- Counsel the patient and family in meeting nursing and related needs.
- Provide health care instructions to the patient as appropriate per assessment and plan.
- Identify discharge planning needs as part of the care plan development and implement prior to discharge of the patient.
- Graduate of an accredited school of nursing.
- One to two years of recent acute care experience in an institutional setting.
- Current licensure in State, CPR certification, valid driver's license and auto insurance.
- Bachelor's degree preferred, with one to three years of home health care experience.
- Management experience not required. Responsible for supervising home health aides.
- Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
$50 - $58 an hour
