Responsibilities- Conduct thorough assessments of residents to identify their care needs and develop individualized care plans.
- Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
- Stay up-to-date with the latest RAI process and PPS requirements to ensure compliance and optimal resident outcomes.
- Provide education and support to caregivers to enhance their knowledge and skills in delivering high-quality care.
- Registered Nurse (RN) with a strong background in gerontology and care management.
- AANC certification (RAC-CT) is highly desirable.
- Experience in MDS nursing and knowledge of the RAI process and PPS requirements.
