Job Title: Physical Therapist

We are seeking a skilled and compassionate Physical Therapist to join our team at Hearts & Hands Post Acute Care and Rehab Center. As a Physical Therapist, you will play a crucial role in helping patients regain their mobility and independence.

Job Summary:

The Physical Therapist will be responsible for evaluating patients' functional needs and developing personalized treatment plans to achieve optimal outcomes. You will work closely with a diverse patient population, utilizing your expertise and skills to provide exceptional care.

Key Responsibilities:
  • Evaluate patients' functional needs and develop personalized treatment plans
  • Provide direct patient care, including therapy sessions and patient education
  • Collaborate with other healthcare professionals to achieve positive patient outcomes
  • Conduct therapy practice within defined standards of care
  • Direct the assistance of support staff to achieve patients' functional goals
Qualifications:
  • Graduate of an approved curriculum in Physical Therapy
  • Current licensure to practice as a Physical Therapist in the state of California
  • Ability to practice unencumbered
  • Working knowledge of Medicare and other payer sources
  • Full knowledge or resident rights
  • Exudes professionalism in presentation
  • Ability to multitask
  • Must be able to read, write, speak, understand and communicate in the English language
