The Rehabilitation Therapist in Home Health is responsible for assessing and evaluating patient care needs related to functional abilities, daily living activities, fine motor skills, home evaluations, adaptive equipment, and other therapy requirements as determined by medical conditions.

Provides services within the legal scope of practice as outlined by state regulations governing Occupational Therapy, in alignment with the care plan, utilizing evidence-based methods, and collaborating with other healthcare professionals.

Evaluates the patient's functional capabilities, assesses all body systems as required for documentation, and identifies occupational therapy needs. Collaborates with the physician to develop the therapy care plan.

Monitors, documents, and communicates to the nurse supervisor and physician the patient's responses to treatment and any changes in their condition.

Conducts thorough patient assessments to evaluate functional levels by applying diagnostic and prognostic tests.


License Requirements:
  • Current Occupational Therapy licensure in the state of practice
  • Current CPR certification
  • Valid driver's license, vehicle insurance, and access to reliable transportation
