Job Summary
The Hospital Social Work Specialist is a critical role at MultiCare, responsible for coordinating patient care across the continuum. This position requires a strong understanding of healthcare systems and a commitment to delivering exceptional patient experiences.
Key Responsibilities
- Perform cross-continuum care activities, including clinic, provider practice, urgent care, and skilled care, as appropriate.
- Provide customer service to patients, families, physicians, and care team members, ensuring seamless transitions of care.
- Model and orchestrate consistent, professional communications with colleagues and staff, maintaining proactive communication patterns.
- Delegate responsibilities appropriately, remaining accountable for outcomes.
- Ensure patients and families experience medically appropriate transitions of care.
Requirements
- Master's degree in Social Work with a healthcare-based practicum.
- Licensed in the state of Washington as a Social Worker Associate Independent Clinical (LSWAIC) with a commitment to obtain LICSW within four years.
- Minimum three years of hospital or equivalent experience.
- Health industry knowledge preferred.
- Healthcare financial assistance knowledge preferred.
