Job Overview:
As a Weekend Registered Nurse, you will work three 12-hour shifts, providing essential nursing care to patients in their homes, following physician orders and adhering to organizational protocols.
Role Responsibilities:
Your primary responsibilities will include conducting initial assessments and developing care plans in collaboration with the Population Health Case Manager. You will also oversee and support Home Healthcare Aides and LPNs as necessary, ensuring comprehensive care management and optimal patient outcomes.
Key tasks include:
1. Implementing treatment plans as per care directives.
2. Utilizing educational resources and adhering to established guidelines.
3. Engaging in quality improvement initiatives to enhance agency performance.
4. Managing ongoing care activities to minimize hospital readmissions and achieve superior patient outcomes.
You will employ motivational interviewing and health coaching techniques to facilitate desired health results. Additionally, you will adjust patient visit schedules to ensure consistent care delivery and operational efficiency, while avoiding overlaps in appointments.
Effective communication with physicians, clinical managers, and family members regarding patient needs and care is essential. You are expected to perform your duties independently and responsibly, maintaining a high standard of care.

Minimum Qualifications:
- Graduate of an accredited Nursing program; BSN preferred.
- Current Registered Nurse license in the relevant state.
- At least one year of professional nursing experience required.
- Experience in Med-Surg, ER, Urgent Care, ICU, Critical Care, or Progressive Care settings.
- Proficient computer and technology skills.
- Valid Driver's license and reliable transportation required.
