Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team. As an LPN at Hermitage Care and Rehabilitation Center, you will play a vital role in delivering exceptional nursing services to our patients and residents. Your primary responsibilities will include:

  • Providing direct patient care and supervision to nursing assistants
  • Developing and implementing individualized care plans to meet the unique needs of each patient
  • Collaborating with interdisciplinary teams to ensure seamless care coordination and communication
  • Monitoring patient progress and adjusting care plans as needed
Requirements

To be considered for this role, you must possess:

  • A current and active LPN license in the state of Kentucky
  • Current/active CPR certification
  • Strong attention to detail and accuracy, with excellent organizational and communication skills
  • Ability to work effectively in a fast-paced environment and prioritize multiple tasks and responsibilities
