Key Responsibilities

As a Certified Nursing Assistant (CNA) at Heritage Home Health, you will collaborate with patients, their families, and a multidisciplinary healthcare team within the patient's residence. Your role will include:

  • Conducting patient visits and ensuring seamless coordination of care.
  • Serving as the primary contact for all healthcare disciplines involved in patient care.
  • Adhering to agency, state, and federal regulations and policies.
  • Consulting with the patient's primary care physician and collecting necessary OASIS information (for Home Health services).
  • Maintaining accurate medication profiles and participating in required meetings.
Qualifications

To be considered for this role, candidates must meet the following criteria:

  • Current CNA licensure/certification in the state of Idaho.
  • Preferred: A minimum of one year of clinical experience.
  • Demonstrated knowledge and proficiency in CNA practices.
  • CPR (BLS) certification.
  • Basic computer skills and the ability to learn industry-specific software.
  • Possession of a valid state driver's license and automobile liability insurance.
