Key ResponsibilitiesAs a Certified Nursing Assistant (CNA) at Heritage Home Health, you will collaborate with patients, their families, and a multidisciplinary healthcare team within the patient's residence. Your role will include:
- Conducting patient visits and ensuring seamless coordination of care.
- Serving as the primary contact for all healthcare disciplines involved in patient care.
- Adhering to agency, state, and federal regulations and policies.
- Consulting with the patient's primary care physician and collecting necessary OASIS information (for Home Health services).
- Maintaining accurate medication profiles and participating in required meetings.
To be considered for this role, candidates must meet the following criteria:
- Current CNA licensure/certification in the state of Idaho.
- Preferred: A minimum of one year of clinical experience.
- Demonstrated knowledge and proficiency in CNA practices.
- CPR (BLS) certification.
- Basic computer skills and the ability to learn industry-specific software.
- Possession of a valid state driver's license and automobile liability insurance.
