Job Summary

We are seeking a highly motivated and detail-oriented Healthcare Advocate to join our team at The Health Plan of West Virginia Inc. As a key member of our D-SNP Program, you will play a vital role in coordinating administrative functions and providing exceptional support to our members.

Key Responsibilities
  1. Health Risk Assessments: Conduct thorough health risk assessments on D-SNP members to identify potential health risks and develop personalized plans to mitigate them.
  2. Member Outreach: Perform outbound outreach calls to welcome new members, close gaps in care, and provide education on health-related topics.
  3. Care Team Coordination: Schedule and facilitate care team meetings, maintain agendas and minutes, and ensure seamless communication among team members.
  4. Resource Referral: Refer members to necessary health services and facilitate access to community resources, transportation services, and personal care services.
  5. Medicaid and Medicare Benefits: Assist members in navigating Medicaid and Medicare benefits, ensuring they receive the care they need.
Requirements
  1. Education: High school diploma or equivalent required.
  2. Technical Skills: Proficiency in basic computer use, keyboarding, and office equipment, as well as knowledge of medical terminology.
  3. Experience: Previous experience in gathering health information via telephonic outreach and working with vulnerable populations.
Preferred Qualifications
  1. Post-Secondary Education: Post-high school coursework in business, social services, healthcare, behavioral health, or healthcare administration is highly desirable.
  2. Strong Communication Skills: Excellent verbal and written communication skills, with the ability to work independently and as part of a team.
  3. Technical Proficiency: Proficiency in word processing programs (Microsoft Word and Excel) and other related PC usage.
