Job Summary:

Aveanna Private Duty Nurse LPN provides high-quality, patient-centered care to medically fragile children and adults in the comfort of their own homes. As a skilled nursing professional, you will work closely with our patients and their families to develop and implement personalized care plans that meet their unique needs and goals.

Responsibilities:

  • Implement and manage patient care plans in accordance with physician orders and company policies
  • Conduct thorough patient assessments and coordinate care with other healthcare professionals as needed
  • Administer medications, treatments, and therapies as prescribed by physicians
  • Document patient care and progress in a timely and accurate manner
  • Provide education and support to patients and their families on health promotion, disease prevention, and management
  • Collaborate with the care team to ensure seamless transitions of care and optimal patient outcomes

Requirements:

  • Active, unencumbered LPN license in the state of practice
  • Current CPR certification from American Red Cross or American Heart Association
  • TB skin test (current within the last 12 months)
  • At least six months of prior hands-on nursing experience (preferred but not required)
  • Reliable transportation
