What You Need to Know
We are seeking a highly skilled and compassionate Registered Nurse RN Case Manager to join our team. As a RN Case Manager, you will be responsible for providing high-quality patient care and managing patient cases in a home health setting.
Responsibilities
As a RN Case Manager, your responsibilities will include:
- Providing patient care and managing patient cases in a home health setting
- Developing and implementing individualized care plans for patients
- Collaborating with healthcare teams to ensure seamless patient care
- Conducting patient assessments and evaluations to identify patient needs
- Developing and implementing patient education programs
Requirements
To be considered for this role, you must have:
- Graduated from an approved school of professional nursing
- Currently licensed to practice as a registered nurse in the state of agency operation
- One (1) year experience as a RN
- Required Certifications and Licensures: Licensed to practice as a registered nurse in the state of agency operation. Must possess and maintain valid CPR certification while employed in a clinical role. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
