Job Summary:
We are seeking an experienced Nurse Practitioner or Physician Assistant to join our Heart Failure Clinic in Denver, CO. As an Advanced Practice Provider, you will partner with our board-certified Heart Failure Cardiologists to build a pre-eminent Heart Failure Program in the state of Colorado.
About the Role:
This is a full-time position that will involve practicing at our Cardiology clinic and Saint Joseph Hospital in Denver, CO. You will work with stage 3 and 4 heart failure patients to ensure medication compliance, monitoring dietary and lifestyle changes, regular follow-ups, and more.
Responsibilities:
- Provide high-quality patient care to stage 3 and 4 heart failure patients
- Monitor and manage patient medication regimens
- Conduct regular follow-ups and adjust treatment plans as needed
- Collaborate with cardiologists and other healthcare professionals to ensure continuity of care
- Participate in inpatient rounding and consults as needed
Requirements:
- Graduate from an accredited Physician Assistant or Nurse Practitioner program
- Licensed as an NP or PA in the state of CO or able to obtain
- DEA and full prescriptive authority in CO or able to obtain
- BLS and ACLS required
- Minimum of 2 years of experience as an NP or PA in a cardiology setting
- Experience in a Heart Failure program, highly desired
