Physical Therapist

About the Role:

We are seeking a skilled Physical Therapist to join our multidisciplinary therapy team in the Millstone, NJ area. As a Physical Therapist with Career Tree Network, you will have the opportunity to work with a well-balanced mix of patients in their homes, providing outpatient therapy in a home-based setting.

Key Responsibilities:

• Provide high-quality, evidence-based physical therapy services to patients in their homes
• Work collaboratively with our multidisciplinary team to develop and implement individualized treatment plans
• Conduct thorough assessments and evaluations to identify patient needs and goals
• Develop and implement treatment plans that promote patient independence and functional ability
• Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination

Requirements:

• Graduation as a Doctor of Physical Therapy (or equivalent standing)
• Eligibility for New Jersey state licensure as a Physical Therapist
• Ability to work a minimum of 12.5 hours per week of direct patient care
• Professionalism, accuracy, and dependability are essential
