Lead the PACE Clinic Program

TidalHealth Orthopedic team in Seaford, DE seeks a Nurse Practitioner to lead the brand-new PACE Clinic program. This high-visibility role will be responsible for pre-operative clearance for knee, hip, and shoulder surgery, focusing on patient engagement, reducing last-minute cancellations, and preparing patients for surgery.

Key Responsibilities
  • Perform histories and physicals for orthopedic total joint replacement patients for the TidalHealth Nanticoke Orthopedic three-person Surgery team.
  • Review preoperative blood work to determine concerning results and address as appropriate.
  • Review OSAP results and prescribe Mupirocin 5 days preoperatively.
  • Assess required Durable Medical Equipment needs and order as necessary.
  • Prescribe postoperative medications in advance of scheduled surgery, including multimodal pain plan, antiemetics, DVT prophylaxis, and stool softeners.
  • Assess physical therapy needs with emphasis on pre-hab and appropriate postoperative referral.
  • Communicate socially complex patients to Case Management and the Inpatient APP team.
  • Schedule radiation with Cancer Center when required for heterotopic ossification prevention.
  • Collaborate with surgeons regarding patients that are medically complex or when additional optimization is needed.
  • Place a preoperative order set that is individualized to patient needs.
  • Cross-train to Inpatient APP role.
Requirements
  • Master of Science in Nursing or Doctor of Nursing Practice and active NP licensure.
  • Current license or ability to obtain a Nurse Practitioner license in Delaware.
  • Previous nursing and/or advanced practice provider experience in Orthopedics is preferred but not mandatory.
  • Comprehensive skills in customer service, clinical, and problem-solving.
  • Demonstrate strong leadership skills and exceptional communication skills in collaboration with other members of the medical staff and administration.
