Main Responsibilities- Coordinate Support: Work closely with individuals, their families, and other stakeholders to coordinate support services that meet their needs.
- Monitor Progress: Regularly monitor the individual's progress, identify areas for improvement, and implement changes as needed.
- Communicate Effectively: Communicate regularly with the individual, their family, and other stakeholders to ensure seamless support.
- Education: High school diploma or equivalent required.
- Experience: Prior experience supporting and working with people with intellectual and developmental disabilities preferred.
- Skills: Excellent communication and organizational skills, ability to understand written and oral directives, and a valid NYS driver's license.
