About the Role

We are seeking a skilled and compassionate Physical Therapist to join our team at Fox Rehabilitation. As a Geriatric Physical Therapist, you will provide evidence-based care to older adults in their homes, helping them to maintain their independence and improve their quality of life.

Key Responsibilities
  • Conduct thorough assessments of patients' physical abilities and develop personalized treatment plans to address their needs
  • Deliver proactive and goal-oriented therapy sessions to patients in their homes, focusing on improving mobility, strength, and functional ability
  • Collaborate with patients' healthcare teams to ensure seamless care and coordination
  • Document patient progress and outcomes in a timely and accurate manner
