Job Summary

The Registered Dietitian serves as a vital member of the PACE Interdisciplinary Team at Element Care. This role works under the direction of the Nutrition Coordinator and the Site Coordinator, providing comprehensive nutrition education and assessments to our PACE participants. As a key member of the IDT, the Registered Dietitian participates in the planning, implementation, and evaluation of care plans that meet the objectives, standards, and policies of the PACE model of care.

Key Responsibilities

• Assess participants' nutritional status at enrollment, annually, and as needed, to ensure optimal care.

• Share relevant participant information with the Interdisciplinary Team to facilitate informed decision-making.

• Collaborate with the team to plan dietary supplements for high-risk participants.

• Report observations and participant changes to ADH staff clinicians, ensuring seamless communication.

• Participate in family meetings and case conferences as needed, promoting a collaborative approach to care.

• Provide follow-up treatment and teaching as necessary, ensuring participants receive comprehensive support.

• Make home visits as requested, demonstrating a commitment to participant-centered care.

• Interact with participants, caregivers, and families in a professional and courteous manner, fostering positive relationships.

• Interact with other providers and community professionals in a courteous and professional manner, promoting effective collaboration.

• Maintain accurate and timely records and documentation, adhering to Element Care's policies and procedures.

• Ability to pass a fit test, with the position requiring a mask with a critical seal, and adhere to the requirement of no facial hair that interferes with the respirator seal.

Additional Responsibilities

• Participate in Quality Improvement projects and assessments as needed, contributing to the continuous improvement of care.

• Participate in Element Care Committees as required, communicating relevant information as necessary.

• Complete required annual Health Stream trainings in a timely manner, ensuring ongoing professional development.

• Adhere to all policies and procedures, supporting Element Care's Primary Vision Statement.

• Maintain confidentiality, adhering to HIPAA guidelines and Element Care's confidentiality policy.

• Demonstrate flexibility in performing other related jobs as reasonably requested.

Requirements

• Master of Science Degree in Nutrition (preferred)

• Registered Dietitian through CDR and current state licensure in the Commonwealth of Massachusetts.

• Two years of experience working in a community setting.

• One year of experience working with the geriatric population.

• Ability to convey information clearly and succinctly with all employees.

• Ability to lead independently and work well with others.

• Ability to work within a team concept and be considerate of co-workers and colleagues.
