The successful candidate will oversee various aspects of the Case Management program, including Remote Patient Monitoring, Transitions of Care, Utilization Management, Risk Stratification, Chronic Care Management (CCM), and Enhance Care Management (ECM). This involves developing work processes and approaches for implementing a centralized Case/Care Management department using nursing standards, Patient-Centered Medical Home concepts, Value-Based Care, and emerging health innovation models.

**Responsibilities:**
  • Develop a comprehensive care plan addressing individual patients' physical, mental, and social needs, reducing barriers to accessing care.
  • Collaborate with a multidisciplinary team to ensure optimal patient outcomes and promote quality of care.
  • Monitor and evaluate the effectiveness of care management activities, modifying processes as necessary.
  • Facilitate complex clinical and financial decision-making by presenting thoroughly analyzed cases requiring higher administrative approval or intervention.
  • Establish and implement integrated continuous quality improvement processes and customer satisfaction initiatives to assure high-quality care and customer satisfaction.
