Care Coordinator for Hospice Patients

Salary Range: $65,000 - $95,000 per yearIn this role, you will work closely with patients and their families to develop personalized care plans and provide high-quality nursing care. You will also collaborate with other healthcare professionals to ensure comprehensive care and support.
Responsibilities:- Conduct initial assessments of patient needs and develop individualized care plans
- Provide direct patient care, including medication administration and wound management
- Collaborate with interdisciplinary teams to coordinate services and ensure seamless care transitions
- Educate patients and families on disease management and self-care techniques
To succeed in this role, you will need:
- A Bachelor's degree in nursing from an accredited institution
- A current state license as a Registered Professional Nurse
- Current BLS certification or willingness to obtain within 30 days
- At least one year of experience in hospice care or a related field
