Job Description

As a Home Health Care Professional - Nursing Excellence at the University of New Mexico - Hospitals, you will play a vital role in providing direct nursing care to patients and families outside the hospital setting. Your primary responsibilities will include:

  1. Assessing patients' physical, psychological, social, and environmental status; developing personalized care plans tailored to their needs.
  2. Providing nursing care and coordinating ancillary services to achieve treatment goals; supervising nursing staff in patient care provision.
  3. Administering medications and performing treatments for assigned patients, in accordance with medical and nursing protocols.
  4. Reporting pertinent observations and reactions regarding changes in patient status to the appropriate team member.
  5. Providing follow-up to ensure problem resolution; recording observations and patient care activities.
  6. Evaluating patients' responses to approved treatment plans; modifying treatment plans as necessary.
  7. Interpreting agency philosophy and policies to patients, families, and other groups; utilizing agency and community resources to benefit clients.
Required Skills and Qualifications

To be successful in this role, you must possess:

  • A minimum of 1 year directly related experience in nursing.
  • CPR for Healthcare/BLS Provider or Professional Rescuer certification within 30 days.
  • RN licensure in New Mexico or through reciprocal agreement.
  • Excellent communication and interpersonal skills.
