Job Description

**About the Role**

We are seeking a highly skilled and compassionate Director of Social Services to join our team at Ely Manor. As a key member of our staff, you will play a vital role in providing exceptional care and support to our residents.

Key Responsibilities

  • Supervise and guide our social workers to ensure the highest level of care and service delivery.
  • Collaborate with facility staff to promote resident advocacy, protection, and rights.
  • Conduct admissions, social services evaluations, and provide referrals as needed.
  • Assist residents with financial and legal matters, such as applying for pensions or referrals to lawyers.
  • Develop and implement discharge planning services to ensure a smooth transition for residents.
  • Provide counseling services and psychiatric medication management as indicated.
  • Ensure accurate and timely documentation in MDS care plans.
  • Supervise and train social workers, including interviewing, hiring, and planning work assignments.

Requirements

  • Education: Bachelor's Degree in Social Work (Master's Degree preferred).
  • Experience: One year of supervised social work experience in a healthcare setting.
  • Certifications: Current Social Work license from the state of MI.
