Key Responsibilities- Deliver comprehensive, home-based medical care to patients with complex needs, working collaboratively with our interdisciplinary teams to develop and implement personalized care plans.
- Conduct initial comprehensive assessments, provide ongoing care, and update care plans as needed to ensure optimal patient outcomes.
- Prescribe diagnostics and interventions to prevent unnecessary acute admissions, and educate patients and their caregivers on disease management and wellness strategies.
- Participate in ongoing meetings and quality initiatives to promote clinical excellence and improve care management and delivery.
- Collaborate with hospital, emergency, and post-acute clinical teams to ensure seamless care transitions.
- Completion of a Nurse Practitioner or Physician Assistant accredited program.
- Current state RN license and NP/PA license.
- Access to reliable transportation.
- 2+ years of clinical experience in primary care, home-based medical care, geriatrics, or palliative care.
- Knowledge of long-term care, community resources, and cost-effective alternatives regarding patient care delivery systems.
- Knowledge of CPT, ICD-10, and HCPC codes.
