We're seeking a dedicated Occupational Therapist to provide high-quality care to patients in our community. Requirements include a graduate degree from an ACOTE-recognized OT program, a valid MN license, and a minimum of 3 years of experience as an Occupational Therapist. Key responsibilities include assessment, planning, and implementation of occupational therapy modalities, as well as developing and updating patient plans of care.   • Qualifications:
  • Graduate degree from an ACOTE-recognized OT program
  • Valid MN license
  • At least 3 years of experience as an Occupational Therapist
  • Criminal background check
  • CPR certification and/or BCLS
  • Active driver's license and clear driving record
  • Computer skills
  • Responsibilities:
  • Assessment, planning, and implementation of occupational therapy modalities
  • Developing and updating patient plans of care
  • Completing patient evaluations and reporting outcomes
  • Leading patients in therapeutic activities to improve functioning and safety
  • Preparing discharge summaries and consulting with family and agency personnel
  • Working Environment:
  • Works indoors in Agency office, patient homes, and travels to/from patient homes
